This should also include cell number changes. This information is so important should we need to reach you regarding your child.
ABSENT CALLS Please remember to call the school when your child will be absent. Even if you let the teacher know about your child’s absence, it is necessary to call the nurse’s voicemail to avoid getting an absent call to verify your child’ s absence. My 24 hour voicemail number is 227-1254. Remember to send a note in to the teacher when your child returns to school. A doctor’s note is need for a 3 consecutive day absence. Feel free to call me anytime with any health concerns about your child throughout the school year. To request homework, call 218-095, enter your child’s teacher’s 4 digit extension and leave a message. Please do not leave this information on the nurse’s voice mail. MEDICATION If your child requires medication during school hours, the proper forms MUST be completed and returned to me with the medication, properly labeled, brought in by an adult. A doctor order and signature as well as the parent signature are required. The district medication policy includes over the counter medications as well as prescriptions. Cough drops are not allowed unless Dr. ordered, since they are a choking hazard in children. SCREENINGS Every child is screened annually for vision, blood pressure, and hearing (no hearing check for 5th grade unless requested). If your child is in need of further evaluation by your physician or optometrist, a referral form will be sent home with your child, for your information. Please have the form completed by your health professional and return it to me. Every child was weighed and measured in Sept. 2003 with the results of 2002/03 sent home in Oct. 2003.
