Student Athlete Participation Information

Dear Parent/Guardian:

Please read the following consent/information forms.  Print and sign the Athletic Participation Permission form and the Athletic Emergency Contact form.  Pre-Participation Physical Packet must be completed by your physician and submitted to the Athletic Office prior to sport participation.
