Medication Administration in School
In order for medication to be administered by a school nurse during school hours, the following conditions MUST be met:
- Medication must be brought into school by a parent/guardian in its original packaging/container. Please ask your pharmacist for an extra bottle of medication needed at school.
- Medication must be clearly labeled with the child's name, drug name, dosage and dose frequency.
- Completed "WTPS Request for Pupil Medication to be given at school" form. This form must have both the parent/guardian and physician signatures. This ensures there is both a physician order and parental consent for the medication to be given in school. This applies to ALL medication, including over-the-counter medications such as Benadryl, cough drops, etc.
- Please call the nurse before bringing in medication to be sure that the nurse is available to take the medication from you. All controlled medications are counted in front of the parent/guardian dropping off the medication.
- Medication Administration Form
