A child who is new to the Washington Township School District is required to provide physical examination documentation within 30 days upon enrollment. A medical exam conducted within 365 days is acceptable and may be used to complete the physical examination form. It is also recommended that a well child recieve a medical exam during each developmental stage [at early childhood (pre-school through grade three), pre-adolescence (grade four through six) and adolescence (grades seven through 12)], if not annually, by their family physician. Please remember to update the Health Office with any current health information, immunizations, and physical findings.
