Some students experience anaphylaxis or a life threatening allergic reaction to certain foods or insect bites/stings. Please let me know if your child has a history of an anaphylactic reaction.
Attached are the forms that will need to be completed by your health care provider. A plan will be developed to protect your child from their allergen. It will also detail how your child would be treated should an exposure occur. This plan will be shared with those who work with your child each day.
