    If your child has a life-threatening allergy and requires medication, such as an anti-histamine or Epi-pen, to be
    available in the School Health Office, the "Life Threatening (Anaphylaxis) Allergy Action Plan" form must be completed by your physician and signed by a parent/guardian. The forms can be returned to the Health Office when the medications are dropped off.  The medication needs to be brought in by an adult to be kept in an unlocked cabinet in the nurses office for emergency use only on your child. An action plan will be developed by the school nurse and signed by the parent. The forms can be found on the WTPS main site or you can contact the nurse and she will email or send them home with your child.
